Uploading photos of your insurance card can be frustrating if you are on a weak cell phone signal
We have setup safe and secure Wifi in the parking lot especially for uploading your documents.
Go to your wifi settings and join the Wifi: CMED-CheckIn
with password: CommunityMed
Step 1 – Shoot Photos
The first thing to do BEFORE HITTING NEXT is to go to your phone camera and shoot a photo of the front and back of both sides of your insurance card(s)
THEN press “Next”

Now press “Select Files”
and pick the ID photo you took from your phone’s gallery

for the front of your insurance card, and pick the insurance card front photo you took from your phone’s gallery, then repeat on the next entry down for the BACK of your insurance card.
