Uploading photos of your insurance card can be frustrating if you are on a weak cell phone signal
We have setup safe and secure Wifi in the parking lot especially for uploading your documents.
Go to your wifi settings and join the Wifi: CMED-CheckIn
with password: CommunityMed
Step 1 – Shoot Photos
The first thing to do BEFORE HITTING NEXT is to go to your phone camera and shoot a photo of the front and back of both sides of your insurance card(s)
THEN press “Next”
Upload Drivers License or ID (Front View)
Upload Drivers License or ID (Rear View)
Upload Primary Insurance Card (Front View)
Upload Primary Insurance Card (Rear View)
Upload Secondary Insurance Card (Front View)
Upload Secondary Insurance Card (Rear View)
Now press “Select Files”
and pick the ID photo you took from your phone’s gallery
